1. What this website is, and is not
This website provides general health information and lets you request care. It is not medical advice, and using it does not create a physician–patient relationship. That relationship begins only when a licensed clinician accepts you as a patient after a visit.
2. Emergencies
Do not use this service for emergencies. Call 911 or go to the nearest emergency department. For a mental health crisis, call or text 988.
3. Eligibility
You must be at least 18 years old and physically located in Illinois, Indiana, or New York at the time of each visit. You must provide accurate information, including your true physical location. Providing false information may make treatment unlawful and will end the relationship.
4. No guarantee of treatment
Paying for a membership or a visit buys a clinical evaluation, not a prescription. A prescription is issued only if a licensed clinician determines it is medically appropriate. If we cannot treat you, we refund you.
5. Cash-pay only
We do not bill any insurer, including Medicare and Medicaid, and we are not enrolled as a participating provider with any plan. You are responsible for all charges. See our Financial Policy.
6. Membership, automatic renewal, and cancellation
Memberships renew automatically each month at the price disclosed at signup until you cancel. You may cancel at any time from your account page; cancellation is effective at the end of the current paid period. There is no fee and no minimum term. We will give advance notice by email of any price change. These terms are provided in accordance with the Illinois Automatic Contract Renewal Act and New York General Business Law § 527-a.
7. Your responsibilities
- Give complete and accurate medical history, including all medications and supplements.
- Complete required laboratory testing and attend required follow-up. We will stop prescribing if monitoring is not completed — this is a safety requirement.
- Report side effects and new symptoms promptly.
- Do not share, sell, or divert any medication prescribed to you. Diversion of a controlled substance is a felony.
- Keep your account credentials secure and enable multi-factor authentication.
8. Secure messaging is not urgent care
Portal messages are answered during business hours, typically within one business day. Do not use messaging for urgent or emergency problems.
9. Intellectual property and trademarks
Site content is owned by Pellmoor Health Technologies, LLC. Third-party medication brand names are the trademarks of their respective owners and are used only to identify the products we may prescribe. We are not affiliated with, endorsed by, or sponsored by any pharmaceutical manufacturer.
10. Recording
We do not record video visits. You may not record a visit without your clinician's consent; several states in which we operate require the consent of all parties to a recording.
11. Termination
Either of us may end the relationship. If we do, we will give reasonable notice, provide emergency prescriptions as clinically appropriate during a transition period, and transfer your records wherever you direct at no charge. We may terminate immediately for abusive conduct toward staff, suspected medication diversion, or material misrepresentation of identity or location.
12. Disclaimers and limitation of liability
The website is provided "as is." To the maximum extent permitted by law, our aggregate liability arising out of your use of the website is limited to the amounts you paid us in the twelve months before the claim. Nothing in these terms limits liability for professional negligence in the provision of medical care, for personal injury, or for any liability that cannot lawfully be limited.
13. Disputes
These terms are governed by the law of the state in which you received care, without regard to conflict-of-law rules. Claims relating to the provision of medical care, including professional negligence claims, are expressly excluded from any arbitration provision and may be brought in court.
14. Changes
We will post changes here with a new effective date and, for material changes, notify active patients by email.
