Grievances
If you believe we have failed to provide these services or discriminated in another way, you may file a grievance with our Privacy Officer at privacy@pellmoorhealth.com or (312) 555-0142. You may file in person, by mail, by fax, or by email, and we will help you if you need assistance.
You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at ocrportal.hhs.gov/ocr/portal/lobby.jsf, or by mail at 200 Independence Avenue SW, Room 509F, HHH Building, Washington D.C. 20201, or by phone at 1-800-368-1019 (TDD 1-800-537-7697).

Effective communication for patients with disabilities
Our video platform supports live captions. We provide written visit summaries to every patient. If you need an ASL interpreter, an assistive listening device, materials in large print, or additional appointment time as an accommodation, tell us when you book and we will arrange it at no cost to you.
