Your Right to a Good Faith Estimate
You have the right to receive a Good Faith Estimate explaining how much your medical care will cost.
- Under the law, health care providers must give patients who do not have insurance, or who are not using insurance, an estimate of the expected charges for medical services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
- Make sure your provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service. You can also ask for one before you schedule.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.

Because we bill no insurance, every patient of this practice is considered uninsured or self-pay for this purpose. We issue your estimate automatically before your first visit, and again whenever your plan of care changes materially. You do not have to ask.

What we do not charge for
- The eligibility check.
- A visit at which we determine we cannot treat you — you are refunded in full.
- Copies of your own medical record, in electronic form.
- Refill requests, prior-authorization paperwork (we file none), or cancellation.
Costs that are not ours
Medication is purchased from a pharmacy of your choosing at that pharmacy's price. Laboratory testing is billed by an independent laboratory. We receive no payment, rebate, commission, or other remuneration from any pharmacy, laboratory, or pharmaceutical manufacturer, and we hold no ownership interest in any of them. Brand GLP-1 medications are expensive without insurance coverage; we will give you a realistic range before you pay us anything.
If you are a Medicare beneficiary
We do not bill Medicare for any service and do not submit claims on your behalf. Medicare will not reimburse you for our fees. Please consider this carefully before enrolling.
Payment, renewal, and refunds
- Payment is by credit, debit, HSA, or FSA card, processed by a PCI-DSS Level 1 processor. We never store your full card number.
- Memberships renew monthly until cancelled. Cancel in two clicks from your account page, any time, with no fee.
- Unused whole months are refunded on request. The month in which you received care is not refunded.
- A failed payment pauses non-urgent scheduling after a 7-day grace period. We will not abruptly discontinue medication over a billing issue — message us.
Superbills
On request we provide an itemized receipt suitable for submission to your insurer or to an HSA/FSA administrator. Whether your insurer reimburses any of it is between you and them.
