Pellmoor exists because the two ends of this market are both bad. Traditional primary care is buried in insurance administration. The new direct-to-consumer prescribers are built to convert, not to treat. We wanted the transparency of the second with the medicine of the first.
How we are structured
Illinois and New York both restrict who may own a medical practice. Only licensed physicians can. That is not a technicality — it exists so that the person deciding whether you get a prescription answers to a medical board, not to a growth target.
So we are two things. Physician-owned professional entities in each state employ the clinicians, hold the medical records, and make every clinical decision. A separate technology and administration company builds the software and handles the non-clinical work under a management services agreement, as a HIPAA business associate.

What we believe
Obesity is a disease, not a character flaw.
It is driven by hormonal regulation of appetite and energy storage. Treating it with medication is not cheating, and needing long-term treatment is not failure.
A narrow scope is a safety feature.
We do two things and refer out for everything else. A practice that claims it can treat anything by video is telling you something about its priorities.
Informed consent is a conversation, not a checkbox.
If a document is written so no one reads it, no one has consented to anything.
Publishing your criteria is a safety feature.
A patient who can read the contraindications before intake is a patient who can catch the one you missed.
The absence of insurance is not a licence to overcharge.
It is a reason to publish every price.
